In our start-up workshop we’ll dig deep into the problem we’re trying to solve. We recommend involving your leadership teams so we can make sure our goals and metrics are aligned with your business strategy.

Develop your healthcare plan

The next step is to roll up our sleeves and co-design a program of healthcare services for your organisation. This will include an agreed pathway for implementation.

It’s important to connect the right person with the right service at the right time. We can support you with communications, templates and training, so you, and your people, can make the most of our services.

Intelligent & integrated care

All of our services are overseen by a Clinical Governance Advisory Group which provides independent advice. This ensures our clinical practice, operations and management are high quality, evidence-based and accountable.

Our model of integrated care

We work in partnership with primary and acute healthcare providers to deliver care that’s proven to work. Our team of over 300 nurses, coaches and allied health professionals look at a person’s physical, emotional and social wellbeing, and give them the tools to take back control of their health.

Reducing health risk

We support and empower people on their health journeys so they can take back control of their health. This enables them to reduce their chronic health risk factors, stay out of hospital and keep their lives on track. 

Staying healthy

Our programs are designed to support individuals as they recover after a health challenge and transition back into everyday life. We help them make long-term behaviour changes, so they can sustain the health they’ve found. 

Treating health conditions

Where possible, we treat our clients at home, in the safety and comfort of their own environment. Our home healthcare services offer an alternative to hospital-based care and are proven to help patients recover faster and keep them well.

Our account managers work with you to continually improve patient outcomes and satisfaction, as well as the financial return for you.

Measuring your impact

In our start-up workshop, we’ll identify what matters most to you and your customers. This will form the basis of what we measure, alongside patient outcomes and experience.

Our evaluation of your healthcare program will drill down into the detail of each service we provide and show engagement, take up, completion rates, clinical outcomes and satisfaction.

HealthierMe is a phone-based health coaching program aimed at supporting your customers with the lifestyle and behavioural changes to prevent, or help cope with, long-term health conditions. All programs are individually tailored and can work alongside a persons current treatment plan.

Our MindStep program is the first of its kind in Australia – a phone-based mental health coaching program using low-intensity cognitive behavioural therapy to help your members manage the symptoms of their anxiety or depression. It focuses specifically on supporting people after they’ve been discharged from hospital.
