We employ a holistic approach to the health of our clients and focus on uncovering what drives a person to better health in the long-term – because evidence shows this helps hospital avoidance.

Our Ethos

We began over 10 years ago with a small group of health coaches dedicated to engaging people in their health. Today, we have over 350 health professionals but still maintain our start-up mentality. Every day, we explore new knowledge, technologies, and collaborations to better integrate our services and support hospitals and GPs. These ongoing conversations keep us passionate and innovative leaders in healthcare.

Engaging, Educating and Empowering

Health coaching is core to all our services. We aim to improve the health, wellbeing, and quality of life of every person we care for by empowering them. Instead of directing, we ask what’s important to them, help set goals, and create plans to achieve them. Our comprehensive approach addresses all aspects of health and wellbeing, reducing risks and keeping people out of hospital while they live life to the full.

Meeting People Where They’re At

We believe some forms of care are best delivered at home. In-home care, including Hospital in the Home, Rehabilitation in the Home, and MindStep, allows people to recover in a comfortable and supportive environment. Our nursing and allied health teams, along with mental health coaches, help clients improve their overall health and prevent hospital re-admissions, easing pressure on the healthcare system.

Challenging Tradition

Our care model is collaborative, involving individuals, hospitals, GPs, community organisations, businesses, and government. This holistic approach integrates care into daily life, promoting long-term health improvements and hospital avoidance. Our dedicated team, including dieticians, nurses, and occupational therapists, empowers people with the knowledge and skills to manage their health, preventing deterioration and unnecessary hospitalisations.

Clinical Approach

As clinicians at heart, our practice is person-centred and evidence-based. Improving health outcomes is our primary goal, supported by functional health, reablement, restorative care, and rehabilitation. Our services are clinically designed and overseen by our Chief Medical Officer and governance team.
